Terms and Conditions

1. By accessing this website or by ordering any product from it you agree to be bound by the terms of this Agreement.

2. By ordering any of the products contained on this website, you agree that the products may only be used for your personal use and may not be sold or redistributed without the written consent of Change Name Pty Ltd (“Change Name”). It is noted that some forms are available from public or governmental bodies for which no claim is made as to ownership therein.

3. Where you have requested Change Name to complete any forms on your behalf, Change Name has completed these forms based on the information that you have provided. Change Name is not responsible for any incorrectly completed forms that arise from either incorrect information supplied by you to Change Name or from information that you have inserted in the forms yourself.

4. In the event of any claim being made against Change Name then the level of damages will not exceed the amount that you paid to Change Name for the provision of the forms.

5. Change Name reserves its rights to reject instructions from any client to provide forms or act on the client’s behalf. Where instructions have already been provided then Change Name reserves the right to terminate the appointment of Change Name. In either of these circumstances Change Name will refund the amount paid by the client.

7. Copyright and all products, information, text, graphic and other material on this website is owned by Change Name with the exception of any forms that are produced by any public or government bodies. You are prohibited from modifying, reproducing, re-supplying, publishing, framing, uploading to a third party, posting, transmitting or distributing the contents in any way without the prior written consent of Change Name. Whenever you reproduce or use any part of this website you must clearly acknowledge Change Name as the source and owner of the copyright.

8. This Agreement is governed by the laws of the State of New South Wales and the Commonwealth of Australia and any dispute will be dealt with in Sydney in the State of New South Wales.
